White "Stereo" print below the centre hole, in a black box.Laminated (> front only) single cover, creases in lamination. Some writing on back (> top & bottom > right > mostly erased). Light creases along the edges. Worn / creased corners, one of them (> bottom > left) being also bent. No "peeping hole" on back.
